2. Please describe your qualifications and/or experience in this field (including student activities, leadership, and volunteer work).
3. What questions do you have for alums and/or employers in this field?
4. Is there anything else you would like us to know/any supplemental information you wish to provide?
5. Please list any dietary restrictions or allergies we should be aware of. 
6. Please list any accommodations needed. 
7. Are you able to commit to attending the full day on Nov. 7th, and do you understand that failure to attend or notify the CDC within 48 hours could result in a loss of funding for future events?
