Hello students,
Welcome to Mount Holyoke! We look forward to serving as your primary care health team while you are here on campus. You will receive a welcome note from your primary campus provider via the patient portal (My Health Connection) sometime in August.
***Thank you to everyone who has already submitted their required health information. If you have fully completed this, you can simply disregard the remainder of this email. Health Services will review your forms and reach out if any required items are missing.***
For those who have not yet submitted the health forms or only submitted portions of the required documents, please read on.
The deadline for completion of Health Requirements is Monday, July 15.
Please do not email to request an extension; we understand that there may be extenuating circumstances that may delay form submission. Complete as much as possible by the July 15th date. There are medical requirements that do not require outside documentation that may be completed at this time online through My Health Connection, the patient portal for students (ie. health history form, TB risk screening, entering the dates of vaccines you know you received) You can update information and submit paper forms or additional documentation at a later date after you obtain documentation or complete a health requirement.
Once forms and dates are entered the status will change to “awaiting review.” Please be patient as your forms are reviewed in the order received, which may take some time due to the volume of submissions. If there are missing requirements after the submissions have been reviewed, we will contact you. You can also check the patient portal for updated status; the status will change from “awaiting review” to “noncompliant” or “compliant.”
Students who are not able to get required vaccinations prior to campus arrival may complete the requirements once here. However, your insurance may not cover the costs of vaccines administered by Health Services.
All students must complete the New Student Health Requirments for undergraduates through My Health Connection; only varsity athletes need to complete the additional Sports Clearance Medical History/Physical Examination Forms and NCAA Forms.
All Students

Health Forms: These are the online Health History and the Consent to Treat Minors form (required only for students less than 18 years old upon matriculation).

Immunization Forms: These must be signed by a medical provider, or you can provide an official copy of your immunization record; students with religious or medical exemptions to required vaccines must complete Exemption Forms.

TB Risk Screening: is required for all incoming students. TB Testing is only required for international students or those who responded “yes” to any of the TB questions in the patient portal.
Athletes

Sports Clearance Medical History and Physical Exam Forms: completed within 6 months of starting sport participation

NCAA Forms: These are the Sickle Test form, the Medication Waiver for banned substances, and the ADHD clearance form (if applicable).

Note: the Health History and Physical Exam forms serve as medical clearance for sports participation.

Completed health forms should be submitted to Health Services by uploading them to My Health Connection. You can log into the platform using your MHC username and password. If unable to upload documentation, you can submit it via these alternative methods:
