Hello students,
Disability Services is the designated office at Mount Holyoke College that provides accommodations and support services to students with documented disabilities for academics, housing, dining and on-campus employment. Students are welcome to register with the office at any point during their time at the College, including when they are on leave or abroad. There is no deadline to request academic accommodations, but often students feel that establishing accommodations and a connection with DS prior to arriving on campus helps to facilitate a smooth transition into the spring semester.
Please review all of the information in this email thoroughly and refer to our guide to applying for accommodations for additional details. This information and other resources related to the office are available on the Disability Services My Mount Holyoke page.
Registering with the office is a three-step process:
-
Submitting a self-report:
-
Login to my.mtholyoke with your MHC credentials
-
Go to the self-service menu
-
Then, click on Students. Under that, is Requests & Applications.
-
-
Providing Documentation from providers
-
In order to provide you with accommodations, appropriate documentation is needed.
-
Please review our Documentation Standards and Requirements to know what type(s) of documentation and information we need for you.
-
We suggest that providers use our Clinician Form.
-
Already existing documentation such as neuro-psych evaluations and previous accommodation records may also be sent in as documentation - sometimes in lieu of a Clinician Form. Please check with Disability Services.
-
-
Documentation can be received by our office via:
-
email to disability-services@mtholyoke.edu
-
fax to 413-538-2827
-
mail to ATTN Disability Services, 50 College Street, South Hadley, MA 01075.

Scheduling an Intake Appointment
-
After we have received your supporting documentation, Disability Services will email your Mount Holyoke College email to schedule your intake appointment with an Accommodation Coordinator. All first appointments with our office are intakes, which typically take an hour.
-
Appointments with Disability Services are available via Zoom or in-person, on the third floor of Mary Lyon Hall. If you are not yet on campus or feeling sick at the time of your appointment, please reach out to switch your appointment to Zoom.

A note about housing accommodations:
To request accommodations related to a disability, you need to go through the accommodations process, described above. For housing accommodations, you need to meet with Disability Services by 12/1/23 to be approved for housing accommodations for SP24. If you are approved, you will fill out a form (in addition to the "New Student Housing Application") letting us know how you would like your accommodations to be implemented. There is no guarantee of placement due to space(s) available.
Due to current scheduling constraints with their office, Disability Services has asked that students get in touch with them as soon as possible regarding housing accommodations.
We are also available for quick questions during our drop-in hours. Our virtual drop-in hours are from 1-2 pm on Tuesdays, and our in-person drop-in hours are from 1-2 pm on Fridays.
Please do not hesitate to call 413-538-2634 or email disability-services@mtholyoke.edu with any questions about any of the above information, or the application process.
On behalf of the entire Mount Holyoke community, #WelcomeMoHome! We can’t wait to meet you.
