Hello students,

 

Welcome to Mount Holyoke!

 

We look forward to serving as your primary care health team while you are here on campus. Keep an eye on your patient portal (My Health Connection) this August, where you will receive a personal welcome note from your primary campus provider.

 

The deadline for completion of Health Requirements is Wednesday, July 15.

 

Already submitted your forms? Thank you! If you have fully completed your health requirements, you can disregard the rest of this email. Health Services is currently reviewing submissions and will contact you directly if anything is missing.

 

Note: Once you upload a document, your status will change to “Awaiting Review.” Please be patient as we review forms in the order they are received.

What if you can't meet the deadline fully?

 

Not submitting everything by the deadline will not prevent you from coming to campus, and you do not need to email us to request an extension. We understand that extenuating circumstances happen.

 

If you are waiting on forms or appointments, please log into My Health Connection and complete what you can. You can log back in to update your information and upload remaining documentation at a later date.
 

 

What you need to submit

All students must complete the New Student Health Requirements for undergraduates through My Health Connection; only varsity athletes need to complete the additional Sports Clearance and NCAA forms.

All students

 

Health forms:

  • Health History

  • Consent to Treatment

  • Minor Consent to Treatment (completed by parent or guardian if you are under 18 years old)

     

Immunization forms:

  • Enter dates of required immunizations

  • Submit documentation to verify (documentation must include name and date of birth, and be translated into English)

  • Acceptable formats:

    • MHC Immunization Form (completed and signed by your medical provider)

    • Official record from your medical provider or former school

    • Your immunization record booklet (translated with name and date of birth)

 

Tuberculosis screening:

  • Tuberculosis Screening Form must be completed by ALL students

  • “Yes” responders will require TB Testing

  • TB Testing Form can be completed by a medical provider

 

Athletes only

 

Sports Clearance Medical History and Physical Exam Forms:

  • Must be completed within 6 months of starting sports participation.

 

NCAA forms:

  • Sickle Test Form

  • Medication Waiver for Banned Substances

  • ADHD Clearance Form (if applicable).

How to submit your forms

Please choose one of the following methods to submit your completed paperwork:

  1. Upload online (preferred): Submit directly via My Health Connection.

  2. Email: Send to medical-records@mtholyoke.edu. Please include your initials and “New Student Health Forms” in the subject line.

  3. Fax: Send to 413-538-2352 (please email health-admin@mtholyoke.edu to confirm the fax was received).

  4. Postal mail: MHC Health Services, Pattie Groves Health Center, 50 College St, South Hadley, MA 01075
